Criteria: Based on education and experience as specified below.
Job Requirements: Provides new parents and agency providers with information regarding the Wraparound process and procedures. Assists in the development of the Strengths, Needs, and Cultural Discover (SNCD) program. Assists the Wraparound Facilitator in conducting the Child Family Team Meetings (CFTM). Participates in CFTM to ensure access, voice and choice within the Wraparound process. Provides a consistent source of encouragement and hope for the family. Demonstrates effective coping techniques for parents and service providers. Documents all activities and interactions with the family. Provides input and represents the family voice in program meetings and system meetings. Provides a variety of practical support to the parents for family activities, home visits, celebrations, etc. Serves as a bridge to enhance parent-professional relationships. Assists in the Family Support Partner training and/or Wraparound 101 training.
Minimum Qualifications: Graduation from high school or equivalent. Successful personal life experiences or involvement with mental health agencies, Department of Human Services and the court system. Former participant in and graduate of the Wraparound process. Completion of the Family Support Partner Training and/or Wraparound 101 Training is preferred. Ability to articulate relevant life experiences to others and to maintain confidentiality.
